The MHNI Phone Nurse will call you within 1-business day during normal MHNI business hours (8:00 a.m. to 5:00 p.m., Monday through Friday) to discuss your issue. Fields marked with an * are required.

Before contacting the Phone Nurse, please review the following to determine if an alternative form needs to be completed:
- If you are an established patient who needs to schedule an appointment at MHNI, please use the Return Appointment Contact Request Form to contact an Appointment Scheduler
- If you have never been seen at MHNI and wish to schedule an appointment, please use the New Patient Headache/Pain Contact Request Form
- Contact the Billing Department if your issue regards billing or payment
